Chapters should complete the Philanthropy Donation form for each philanthropic donation/fundraiser. Chapters should complete the Community Service Report form for each service activity.

Frequently-used Reporting Forms
- Bid Acceptance Agreement Form: Must be submitted by each new member within 48 hours of accepting a bid (invitation to membership)
- Change in Status Form: Must be submitted within 48 hours of a membership status change
- Safe Social Hosting Event Registration Form: Required for each event in which alcohol will be present; submission of form does not guarantee the event will be approved. See Safe Social Hosting Procedures (page 55).
- Chapter Information and Officer Update Form: To be submitted by chapter after officer transitions to update all chapter officer information
- Individual Chapter Contact Update Form: To be submitted only when one or two officers change mid-term
- Advisor, House Corporation, and Headquarters Contact Update Form: To be submitted by chapter/advisor/HCB to update chapter officer information
Housing, Financial & Meal Forms
- Fire Drill Checklist: Due upon completion of a fire drill, at minimum once per semester
- Housing Closing Form: Due at the end of spring semester
- Chapter Financial Profile Form ("billing roster"): Due to Student Financial Services by a designated date each semester for the following semester's billing

- Fraternity/Sorority Interest Form - Submit this form if you want to express interest in one or more chapters; the form information will be sent to the chapter(s) you select.
- Academic Grade Release Request - Allows a prospective member to release their academic information to a chapter for the purposes of determining membership eligibility
